ENROLLMENT AND TUITION POLICY
1. Enrollment: registration
Welcome to Ustar Education Learning Center After School Program! We charge a $50 non-refundable registration fee for each new enrolled student, $35 for returning student.
2. All Students: Annual Enrollment Deposit: $300
This payment indicates your intention to enroll and secure a spot at Ustar Education. For new students, the $300 enrollment deposit will be used as the One-Time After-School deposit ($300) when the school year begins in the fall. For existing students, it will be applied as a credit towards your tuition for the fall semester.
Please note that if a student does not attend Ustar Education in the Fall, the Annual Enrollment Deposit ($300) will not be refunded.
3. Space Availability
Ustar Education provides a registration waiting list when the program is full, and enrollment will be in order of registration.
4.Sibling Discount
The second child in the family enjoys a 5% sibling discount (for 5-day after school program only).
5..Material Fee
The tuition fee above does NOT include material fee. $5/week material fee is required for each enrolled student. Material fee covers (not limited to) daily worksheet copies, stationery, and bonus gifts for good performance, but NOT includes textbook fees. The tuition fee and material fee above does NOT include daily pick up fee.
6. Holiday Breaks
Ustar Education is closed on the following holidays: Martin Luther King Jr Day, President’s Day, Spring Break (2 days), Memorial Day, Labor Day, Thanksgiving, Winter holiday, Christmas, and New Year.
7. Absences
If the child is absent for a lengthy period (for any reason), parents can choose to either pay a full month’s tuition to keep the enrollment status or temporarily or permanently withdraw from the program.
8. Schedule Changes
For any schedule changes from the after-school program, parents will need to notify Ustar Education at least one month in advance.
9. Tuition Deadline
Tuition is due on the 6th day of each month, and is delinquent after the 6th day of the month. There is a 10% late charge for any payment received after the 6th of the month. The date we receive the payment is defined as the payment date, not the date shown on the payment check.
10. Late Pick-up Penalty
If a child is picked up later than the scheduled pick-up, parents shall be assessed a fine of $1/minute after the scheduled pick-up time.
11. Discount
For student who enrolled the one school year, can get the special discount. Enrolled 10+ classes per week, get 50% discount, 8+ classes, per week get 40% discount, 6+ classes per week, get 20% discount, 4+ classes per week, get 10% discount, 2 classes per week get 5% discount. If student withdraw the classes, the discount will change based on the enrolled total classes per week after next week when request received. If student withdraw from school, the discount won’t apply. Need recalculate the classes based on the original price.
12. Withdrawal from the Program
Parents requesting a withdrawal (temporary or permanent) must complete the student withdrawal form one month in advance to inform Ustar Education of the student’s withdrawal. If the form is not completed one month in advance, the deposit amount is not issued. If student enrolled one school year to get the discount, and withdraw earlier, the deposit amount is not issued, or student need pay the classes original price.
Temporary Withdrawal – Absence NO longer than one month qualifies our temporary withdrawal. Parents may retain their deposit. We ask for 2-week tuition in their Ustar Education Account to save the space until the specified return date. If the student does not return on the specified return date, the tuition amount will be deducted from the deposit.
Permanent Withdrawal – Parents may apply their deposit to pay remaining tuition. For permanent withdrawals, there will be a $35.00 re-registration fee, if the student returns to Ustar Education in the future.
If you choose to enroll the full semester, withdraw earlier, the security deposit is nonrefundable and can’t be used for deducting last month’s tuition.
13. Method of Payment
Unless otherwise expressly provided in this agreement, any payment made by the parents to the center must be made by cash, check (title: USTAR EDUCATION LLC), or Zelle (onsite@loveustar.com) payable to USTAR EDUCATION LLC. There is a $50.00 fee for any returned checks. If this should occur more than once, future payments must be made only in cash or money orders.
14. Termination Conditions
Ustar Education may terminate a child’s enrollment in the program effective immediately upon written notice to Parent of such termination, if any of the following conditions arise:
(1) In the sole judgment of the Director, a child’s behavior threatens the physical or mental health or well-being of one or more of the other children at USTAR EDUCATION.
(2) Parent is hostile, uses profane language, is verbally or physically abusive or threatening to staff or clients of the facility.
(3) Any payment owed by Parent to Ustar Education is not paid within one month due.
(4) Child is picked up late more than three (3) times during any month.
(5) Parent fails to pick up child promptly when notified child is ill.
(6) Non-cooperation of Parent. Parent refuses to comply with agreed upon plan of action relating to child’s behavior.
STUDENT WAIVER & RELEASE OF LIABILITY
Student Waiver & Release of Liability
Ustar Education, LLC (also referred to as “Ustar Education and/or USTAR) provides a world-class performing class environment for all age groups. We believe that the quality of our environment enriches the lives of our students, encouraging a powerful outlet for freedom of expression, creativity, and confidence. We also support a team approach that fosters commitment to the team yet allows each student to achieve his/her personal best. We are committed to providing the best possible learning experience for all of our students. But most importantly, we want our students acquire academic knowledge and skills.
Assumption of the Risk and Waiver of Liability Relating to Coronavirus/COVID-19
The novel coronavirus, COVID-19, has been declared a worldwide pandemic by the World Health Organization. COVID-19 is extremely contagious and is believed to spread mainly from person-to-person contact. As a result, federal, state, and local governments and federal and state health agencies recommend social distancing and have, in many locations, prohibited the congregation of groups of people.
Ustar Education has adopted guidelines and protocols promulgated by responsible governmental agencies as preventative measures to reduce the spread of COVID-19; however, Ustar Education cannot guarantee that you or your child(ren) will not become infected with COVID-19. Further, attending Ustar Education could increase your risk and your child(ren)’s risk of contracting COVID-19.
By signing this agreement, I acknowledge the contagious nature of COVID-19 and voluntarily assume the risk that my child(ren) and I may be exposed to or infected by COVID-19 by attending Ustar Education and that such exposure or infection may result in personal injury, illness, permanent disability, and/or death. I understand that the risk of becoming exposed to or infected by COVID-19 at USTAR may result from the actions, omissions, or negligence of myself and others, including, but not limited to, USTAR employees, volunteers, and program participants and their families.
Student Waiver
I understand and acknowledge that the programs provided by Ustar Education involve physical activity that involve inherent risks that may result in harm to the participants. I voluntarily agree to assume all of the risks and accept sole responsibility for any injury to my child(ren) or myself (including, but not limited to, personal injury, disability, and death), illness, damage, loss, claim, liability, or expense, of any kind, that I or my child(ren) may experience or incur in connection with my child(ren)’s attendance at Ustar Education and/or participation in Ustar Education classes.
On my behalf, and on behalf of my children, I hereby release, covenant not to sue, discharge, and hold harmless Ustar Education , its employees, agents, owners, and representatives, of and from any claims, including all liabilities, claims, actions, damages, costs or expenses of any kind arising out of or relating thereto. I understand and agree that this release includes any claims based on the actions, omissions, or negligence of USTAR, its employees, agents, and representatives, whether a COVID-19 infection occurs before, during, or after participation in any Ustar Education program.
I understand that Ustar Education LLC will make a reasonable attempt to contact me as soon as possible in the event of a medical emergency involving my child. I agree to provide Ustar Education LLC with current contact information and acknowledge that it is my obligation to promptly notify Ustar Education with any changes or updates to that information. In the event that I am unable to be reached during an emergency, I authorize Ustar Education, in its sole discretion, to contact emergency services for appropriate treatment and care, and further, I assume the responsibility for the payment of any such care and treatment.
Releases
I agree to participate and/or authorize my child to participate in classes offered by Ustar Education LLC and hereby release and indemnify Ustar Education LLC, its owners, managers, affiliates, employees, instructors and volunteers from any liability associated with these classes. This paragraph should be understood as a complete release of liability associated with my and/or my child’s voluntary participation in USTAR classes.
I understand and agree that Ustar Education LLC may, at its sole discretion, record the classes in which my child and/or I participate. I hereby grant permission to the rights to my image, likeness and sound of my voice, and those of my child, as recorded on audio or video tape. I understand and agree that Ustar Education LLC may use any such recordings, audio or visual, in all media, including, but not limited to, print, video, digital and internet media, for advertising, trade and any other lawful purpose and that these recordings may be used without further notification to me. I acknowledge and agree that Ustar Education LLC exclusively owns all rights to the images, videos, and recordings, and to any derivative works created from them. I further acknowledge and agree that I will not be compensated for these uses, and I waive any right to royalties or other compensation arising out of or relating to the use of these recordings. I understand that my child’s name may not be used without my permission.
By submitting my personal information to Ustar Education, I agree that in the event of a merger, acquisition, reorganization, sale of all or substantially all of its assets, or the sale of an individual website owned by Ustar Education LLC, Ustar Education LLC may transfer my information to a third party as a part of such merger, acquisition, reorganization, sale, or other future business transaction.
I acknowledge that I have completely read and fully understand the above release and agree to be bound thereby. This Release is binding on me, my heirs, assignees, other guardians of my child and estate.
I have had sufficient opportunity to read this entire document. I have read and understood it, and I agree to be bound by its terms.
Parent(s) or Court-Appointed Legal Guardian(s) must sign for any participating minor (those under 18 years of age) and agree that they and the minor are subject to all the terms of this document, as set forth above. I certify that I am the Parent or Legal Guardian of the above minor.
