Refund Policy

After first official class, no refund will be issued. Remaining class credits can be used for other group class. 

Make-up Policy

During the whole course session, allow 2 make-up classes in the same week other time if there is the same level class. If not, half of the class credit will be issued. Credit can use for other group class.

Student Waiver & Release of Liability

Ustar Robotics LLC (also referred to as “Ustar Robotics “) provides a world-class performing class environment for all age groups.  We believe that the quality of our environment enriches the lives of our students, encouraging a powerful outlet for freedom of expression, creativity, and confidence.  We also support a team approach that fosters commitment to the team yet allows each student to achieve his/her personal best.  We are committed to providing the best possible learning experience for all of our students.  But most importantly, we want our students acquire academic knowledge and skills.

Assumption of the Risk and Waiver of Liability Relating to Coronavirus/COVID-19

The novel coronavirus, COVID-19, has been declared a worldwide pandemic by the World Health Organization. COVID-19 is extremely contagious and is believed to spread mainly from person-to-person contact. As a result, federal, state, and local governments and federal and state health agencies recommend social distancing and have, in many locations, prohibited the congregation of groups of people.

Ustar Robotics has adopted guidelines and protocols promulgated by responsible governmental agencies as preventative measures to reduce the spread of COVID-19; however, Ustar Education  cannot guarantee that you or your child(ren) will not become infected with COVID-19. Further, attending Ustar Education could increase your risk and your child(ren)’s risk of contracting COVID-19.

By signing this agreement, I acknowledge the contagious nature of COVID-19 and voluntarily assume the risk that my child(ren) and I may be exposed to or infected by COVID-19 by attending Ustar Education  and that such exposure or infection may result in personal injury, illness, permanent disability, and/or death. I understand that the risk of becoming exposed to or infected by COVID-19 at USTAR may result from the actions, omissions, or negligence of myself and others, including, but not limited to, USTAR employees, volunteers, and program participants and their families.

Student Waiver

I understand and acknowledge that the programs provided by Ustar Robotics involve physical activity that involve inherent risks that may result in harm to the participants.  I voluntarily agree to assume all of the risks and accept sole responsibility for any injury to my child(ren) or myself (including, but not limited to, personal injury, disability, and death), illness, damage, loss, claim, liability, or expense, of any kind, that I or my child(ren) may experience or incur in connection with my child(ren)’s attendance at Ustar Robotics and/or participation in Ustar Robotics classes.

On my behalf, and on behalf of my children, I hereby release, covenant not to sue, discharge, and hold harmless Ustar Robotics , its employees, agents, owners, and representatives, of and from any claims, including all liabilities, claims, actions, damages, costs or expenses of any kind arising out of or relating thereto. I understand and agree that this release includes any claims based on the actions, omissions, or negligence of USTAR, its employees, agents, and representatives, whether a COVID-19 infection occurs before, during, or after participation in any Ustar Robotics program.

I understand that Ustar Robotics LLC will make a reasonable attempt to contact me as soon as possible in the event of a medical emergency involving my child. I agree to provide Ustar Robotics LLC with current contact information and acknowledge that it is my obligation to promptly notify Ustar Robotics with any changes or updates to that information. In the event that I am unable to be reached during an emergency, I authorize Ustar Education, in its sole discretion, to contact emergency services for appropriate treatment and care, and further, I assume the responsibility for the payment of any such care and treatment.

Releases

I agree to participate and/or authorize my child to participate in classes offered by Ustar Robotics  and hereby release and indemnify Ustar Robotics , its owners, managers, affiliates, employees, instructors and volunteers from any liability associated with these classes.  This paragraph should be understood as a complete release of liability associated with my and/or my child’s voluntary participation in USTAR classes.

I understand and agree that Ustar Robotics may, at its sole discretion, record the classes in which my child and/or I participate.  I hereby grant permission to the rights to my image, likeness and sound of my voice, and those of my child, as recorded on audio or video tape. I understand and agree that Ustar Robotics may use any such recordings, audio or visual, in all media, including, but not limited to, print, video, digital and internet media, for advertising, trade and any other lawful purpose and that these recordings may be used without further notification to me.  I acknowledge and agree that Robotics exclusively owns all rights to the images, videos, and recordings, and to any derivative works created from them.  I further acknowledge and agree that I will not be compensated for these uses, and I waive any right to royalties or other compensation arising out of or relating to the use of these recordings.  I understand that my child’s name may not be used without my permission.

By submitting my personal information to Ustar Robotics , I agree that in the event of a merger, acquisition, reorganization, sale of all or substantially all of its assets, or the sale of an individual website owned by Ustar Robotics ,  may transfer my information to a third party as a part of such merger, acquisition, reorganization, sale, or other future business transaction.

I acknowledge that I have completely read and fully understand the above release and agree to be bound thereby.  This Release is binding on me, my heirs, assignees, other guardians of my child and estate. 

I have had sufficient opportunity to read this entire document. I have read and understood it, and I agree to be bound by its terms.

Parent(s) or Court-Appointed Legal Guardian(s) must sign for any participating minor (those under 18 years of age) and agree that they and the minor are subject to all the terms of this document, as set forth above. I certify that I am the Parent or Legal Guardian of the above minor.
